Richard's a dead man walking. Dead inside too. Andrew is a dead man walking. Sonny's gang are all dead men walking and they're dead inside. The titular shoes belong to those who are on a journey to their deaths.

If I recall correctly one of the early working titles was either 'Dog Altogether' or 'Skull'

I always had an idea that 'Dead Man's Shoes' came from the position of Sonny. The expression 'in their shoes' meaning to sense what somebody else feels or felt has something to do with my idea.
Just as how Anthony was tortured by monsters Sonny is now being a victim or paranoia and torture. This is placing him in Anthony's position. He is in Anthony's shoes...a dead man's shoes.
...Does that make any sense?...

Granted, if that were the case, the "twist" that Anthony is actually dead would be right there in the title; but for me, the real twist is Richard's motivation for going after Sonny's gang -- not just a case of "an eye for an eye," but because of his own guilt about having abandoned Anthony out of embarrassment in the first place.
